1.0 Summary

1.1. Purpose: To procure consultancy services to conduct Comprehensive Community Tuberculosis (TB) Community Rights and Gender Assessment (CRGA) using STOP TB assessment tools.

**1.**2 Partners: Global Fund, Kenya Red Cross Society (KRCS), Tuberculosis Leprosy and Lung Disease Program (NTLD), AMREF Health Africa, STOP TB Partnership Kenya, and county governments.

1.3 Target population: People infected and affected by TB.

1.4. Estimated dates: 14th November to 2nd December 2022

1.5. Duration: 15 days

1.6. Deliverables: Inception and Final assessment report.

1.7 Methodologies: A cross-sectional study, using a mixed-method approach that includes qualitative and quantitative methods.

1.2 Background Information

Kenya Red Cross Society (KRCS) is the non-state Principal Recipient (PR) for the Global Fund HIV Grant for the period between July 2021 and June 2024. The goal of the grant is to contribute to attainment of universal health coverage through comprehensive HIV prevention, treatment and care for all people in Kenya with the objectives to reduce new HIV infections by 75%, reduce AIDs related mortality by 50% and reduce HIV related stigma and discrimination to less than 25%.

Tuberculosis remains a major public problem, and there are still a lot of challenges faced as we near ending TB globally by 2030. An estimated 10 million people fell ill (1.1 million children), and 1.5 million died from TB in 2020 despite TB being preventable, treatable, and curable. Kenya is among 30 nations with a high TB disease burden that accounts for 86% of the world’s TB cases. TB is the fifth leading cause of death in Kenya. Tuberculosis (TB) thrives in conditions of structural inequity, where the complexities of poverty, social inequity, disempowerment, rights violations, conflict, and patriarchy render communities susceptible to TB and marginalize access to diagnosis, treatment, and care. The 2030 Agenda for Sustainable Development emphasizes that its Sustainable Development Goals (SDGs) will not be achieved unless and until human rights and dignity are ensured for all individuals, everywhere, leaving no one behind. Gender assessment conducted by Stop TB Partnership (Building the evidence for a Rights-Based, People-Centered, Gender-Transformative Tuberculosis Response”) shed new light on the role of gender in the TB epidemic and response.

Kenya has progressively used evidence-based approaches in combating TB which has seen various strategies adopted for implementation and inform policy. The most identified human rights-related barriers to ending the TB epidemic, limiting access to TB services, and negatively impacting the quality of life are stigma and discrimination. As such, it is critical to understand the levels and dimensions of TB stigma and develop evidence-based strategies and actions to address it. This will in return reduce people's vulnerability to TB infection, increase people's access to TB services, and improve TB treatment outcomes.

The Stop TB Partnerships highlights the need to address human rights, vulnerable populations, and gender in order to control the TB epidemic. Similarly, The Global Fund's response to the TB epidemic encourages programs to remove human rights and gender-related barriers to TB prevention, diagnosis, and treatment services. The Global Fund's new Strategy 2017-2022: Investing to End Epidemics Strategic Objective 3 (SO3) states that "Promoting and protecting human rights and gender equality.

In response to TB, Global funds through KRCS is implementing the Reducing human rights-related barriers to HIV/TB services module in 46 counties of which Conducting Comprehensive TB Community Rights and Gender Assessment STOP TB assessment tools is one of the activities under the module.

1.3 Consultancy

KRCS seeks the services of a consultant to conduct Comprehensive TB Community Rights and Gender Assessment using STOP TB assessment tools. The Consultant will review and finalize the protocol and data collection tools, support and guide the TB stakeholders in conducting, community rights and gender assessments including training of the research assistants on the data collection tools, data analysis, report writing, dissemination, and development of key action plans for the comprehensive TB and community rights and gender assessment and TB stigma assessment.

2.0 The assessment, Purpose, Scope, and Objectives

2.1.1 Purpose

The program has planned to conduct Comprehensive TB Community Rights and Gender Assessment using STOP TB assessment tools to describe the policy and legal framework in relation to community rights and gender (CRG) and TB in Kenya, describe the gender responsiveness in TB programming, and assess the uptake and utilization of TB services among the TB Key and vulnerable populations in relation to community rights and gender.

2.1.2 Broad Objectives

The objective is to assess the responsiveness of TB services with regard to Community Rights and Gender (CRG) in Kenya.

2.1.3 The Specific Objectives of the consultancy include the following:

  1. To describe the policy and legal framework in relation to community rights and gender (CRG) and TB in Kenya.
  2. To describe gender responsiveness in TB programming.
  3. To assess the uptake and utilization of TB services among the Key and vulnerable populations in relation to community rights and gender.

2.1.4: Outcomes

  1. Policy and legal environment frameworks currently available in relation to CRG and TB in Kenya.
  2. The extent to which TB programming is responsive to gender dynamics in Kenya.
  3. Uptake and utilization of TB services among the Key and vulnerable populations in Kenya.

3.0 Scope of work

3.1.1 Geographical coverage: Nairobi, Narok, Murang’a, West Pokot, Kisumu, Kakamega, Garissa, Isiolo, Mombasa, Migori and Kitui

3.1.2 Target groups: Individuals who have had Drug - sensitive TB (DSTB) and/or Drug-resistant TB (DRTB), Close family members and neighbors – linked to TB patients currently on treatment, The community – schools and learning institutions/prisons/Homeless persons/Administration (chiefs’/village elders)/Drug and substance Users/Transgender, Health Care Workers – clinicians, nurses, Community Volunteers, and link assistants.

      1. Expected outputs:

3.1.3.1 A technical and financial response to the call for applications outlining the understanding of the task, detailing the appropriate methodology for the assessment, data analysis plan, work plan with tentative timelines and summary budget.

        1. An inception report of the desk review prior to field work to demonstrate a clear understanding and a practical work plan for the assessment 2. Submission of the final baseline assessment report to the evaluation team, including raw, final databases and any recorded material such as video, audio etc. 3. Delivery of a final costed Community rights, Gender assessment and TB Stigma Reduction Action Plan for TB Control.

3.1.4 Deliverable

The assessment will be phased with deliverables at intervals that will be discussed and agreed with the consulting firm. However, at the minimum

4.0 Evaluation methodologies

KRCS highly recommend that the consultancy considers effective approaches and design that responds to the assessment objectives and scope of this assignment. However, additional and effective approaches may be recommended to meet the data requirements for the assessment as agreed with EMT. The applicant is required to recommend sampling techniques for the assessment in their technical proposal, which adheres to the laws of Statistical Regularity and Inertia of large numbers. The data collection and analysis methods must be comprehensively and explained in the technical document.

5.0 Quality & Ethical Standards

The applicant is required to outline quality and ethical standards in the proposal and ensure that the assessment is designed and conducted to respect and protect the rights and welfare of the vulnerable population. The findings of the assessment will be expected to be technically accurate and reliable; is conducted in a transparent and impartial manner. The firm may be required to seek ethical approval prior to the commencement of the data collection process. In addition, the team will be required to adhere to the research standards and applicable practices as recommended by MOH, International Federation of Red Cross and Red Crescent Societies.

5.1 Utility: Survey must be useful and used.

    1. Feasibility: The baseline must be realistic, diplomatic, and managed in a sensible manner.

5.3 Ethics & Legality: Survey must be conducted in an ethical and legal manner, with particular regard for the welfare of those involved in and affected by the evaluation.

    1. Impartiality & Independence: Baseline should be impartial, providing a comprehensive and unbiased assessment that takes into account the views of all stakeholders.
    1. Transparency: Survey activities should reflect an attitude of openness and transparency.
    2. Accuracy: Baseline survey should be technical accurate, providing sufficient information about the data collection, analysis, and interpretation methods so that its worth or merit can be determined.
    3. Participation: Stakeholders should be consulted and meaningfully involved in the evaluation process when feasible and appropriate.

6.0 Qualifications and experience of the evaluation Firm

    1. Master’s Degree in Public health, Social Sciences or a related field.

6.2 Minimum of 7 years of research, program planning experience in large, health sector development programs that have implemented successful activities.

    1. Demonstrated expertise in all aspects of TB, community rights and gender, Public Health, advocacy and/partnerships for health
    2. A clear track record in provision of technical assistance/consultancy of similar complexity and magnitude.
    3. Demonstrated ability to build and maintain relationships with senior-level Government of Kenya officials, NGOs, private sector, development agencies, implementing partners and donor organizations.
    4. Demonstrated strength in developing and maintaining stakeholder partnerships and engagements
    5. Professional proficiency in English is required.
    6. Excellent analytical (qualitative and quantitative), presentation.
    7. Ready to carry out the assignment and deliver results within a short time.
  2. Management of the Assessment

7.1 Duration: The entire survey will take a maximum of 15 working days. The scheduled start date is on 14th November 2022 and end date is on 2nd December 2022.

    1. Requirements:

7.2.1 Inception Report should be submitted in soft copy.

7.2.2 All hard copy forms for data submission and soft copy in a flash disk.

7. 2.3 Soft copy Draft Evaluation Report, editable for review and inputs.

7.2.4 Final Evaluation Report, well visualized – 10 hard copies (Gloss paper and perfect book bound) and soft copy in a Flash Disk, including the PowerPoint presentation used during disseminations.

7.3 Roles of Evaluation Management Team: The project management team will include KRCS Program and Monitoring and evaluation (M&E) team, Tuberculosis Leprosy and Lung Disease Program (NTLD), AMREF Health Africa and STOP TB Partnership Kenya. The role of the team is envisaged but not limited to the following:

7.3.1 Development and finalization of the baseline survey Terms of reference.

7.3.2 Procure the consulting firm and provide logistical support during the survey process.

      1. Provide Consulting team with the relevant documents as required.
      2. Plan for the report validation meeting with key stakeholders.
      3. Review and approve the survey documents and the final report.
      4. KRCS to make fund available to the consultant as per the contract document

8.4 Role of the Consultant:

Respond to the TOR highlighting the understanding of the tasks, proposed and detailed methodology, work plan and budget.

Undertake the desk review and develop literature review.

      1. Develop inception report, review the protocol and data collection tools.
      2. Sensitization of research data assistants for data collection data analysis and report writing
      3. Work closely with the KRCS and National TB program and the stakeholders to identify and review all existing sources of strategic information towards conducting Community rights and gender assessment and TB stigma assessments.
      4. To support the planning and implementation of inclusive dialogue processes, including the provision of advice on relevant strategies for stakeholder engagement and, where requested, provision of facilitation support.
      5. To provide advice and support to the stakeholders on participatory approaches and engagement of community, community actors, CSOs, implementing partners and other relevant stakeholders.
      6. To ensure that inputs from TB community, community actors, CSOs, implementing partners, consultations, dialogue forums, and stakeholders are solicited and incorporated into findings of the assessments.
      7. To draft a costed Community rights, Gender and TB Stigma Reduction Action Plan for TB Control in collaboration with the stakeholders, coordinating inputs from all sources.
      8. To prepare/gather and incorporate all needed annexes in the context of community rights, stigma and gender in TB control
      9. To facilitate timely peer review of the first draft and to work with technical teams to incorporate recommended revisions.
      10. Ensure all editorial checks are completed to ensure all sections are consistent, organized and prepared in a timely manner.
      11. To facilitate dissemination of the findings, reports and costed action plan for community rights, gender and TB stigma assessment Plan.
      12. Develop and submit the final evaluation report.
      13. Develop study dissemination pack and facilitate actual dissemination.
      14. Share all the data sets with KRCS and the evaluation management team

9 Submission of Proposal

All Bidders are required to meet the following minimum requirements in order to qualify for submission of their proposal:

    1. Previous experience in similar assignments.
    2. Capability of the assessment team: Team member(s) shall relevant experience as stated above.
    3. Attachment of all supportive documents with the technical proposal is required.

  2. Application materials shall include:

    1. A written response to this TOR in terms of a proposal detailing the technical understanding of the task, proposed methodologies of execution of the assignment, expected activities and deliverables, proposed work plans with schedule, and financial bids. See Annex 1.
    2. Detailed CVs of all professional (s) who will work on the assignment. If there is more than one consultant on the proposed team, please attach a table describing the level of effort (in number of days) of each team member in each of the assignment activities. See Annex 3
    3. . Professional references: please provide at least three references from your previous clients and full contact details of the referees

The Technical Proposal MUST be prepared in conformance to the outline provided in Annex 1 while the financial proposal shall conform to the template provided in Annex 2. Team composition should conform to Annex 3

ANNEX 1: RESPONSE PROPOSAL FORMAT

  1. Introduction: description of the firm, the firm’s qualifications and statutory compliance (Max 2 pages)
  2. Background: Understanding of the project, context and requirements for services, (Max 2 pages)
  3. Proposed methodology: The proposed methodology MUST provide an indication of its effectiveness and added value in the proposed assignment. Describe the proposed design for the assessment. Indicate methods to be used for each objective and highlight any areas where adjustment may be recommended. The targeted respondents should be indicated for each objective. Detailed sample calculation methods and sampling procedure needs to be indicated (Max 6 pages)
  4. Firms experience in undertaking assignments of similar nature and experience from the geographical area for other major clients (Table with: Name of organization, name of assignment, duration of assignment (Dates), reference person contacts-2 pages
  5. Proposed team composition (As per annex 3)-1 page
  6. Work plan (Gantt chart of activity and week of implementation)-1 page
  7. Recommendation letters for previous similar assignment.
